Alsamixer has all settings on nothing is muted. However, when I play a test wav file using aplay nothing is heard although aplay does not report any problems.
lsmod shows that the drivers are loaded.

One thing I noticed is that the function key Fn+Esc which is supposed to mute/unmute sound on this Toshiba Portege R200 does not function. xev program does not record any keystrokes when Fn+Esc is pressed. However, if keyboard has muted the sound card one would think that it would appear in the sound applet which shows that it is in unmuted state.
Any ideas or suggestions will be appreciated.

Not sure the importance of this but I noticed that when run aplay the file gets played at a sampling rate of 88200 although it has a sampling rate of 44100.

Finally had a breakthrough....
In alsamixer there was a control called 'Head Phone Jack Sense' - just muted it and now I can playback sounds without any issue. A bit strange but hey, it works!

Thanks for reporting your resolution.
Head Phone Jack Sense should only disable laptop speakers and/or speaker output port when a 3.5mm headphone jack is plugged into the laptops headphone port.

Note
On some models (T40, T43p, ... R51e ...) audio is muted if either Headphone or Line Jack are unmuted. See also ALSA Wiki FAQ [1]. Also, if Headphone Sense or Line Jack Sense are unmuted audio is dead (R50e, R52, X40). By default the Sense settings are not shown in mixer.

For the benefit of others who may stumble across this issue, the laptop in question is a Toshiba Portege R100. Note that both Headphone Jack Sense and Line Jack Sense had to be muted to get audio working.
